Association between lower esophageal sphincter resting pressure and esophageal motor disorders

Background: One of the most important factors involved in the pathophysiology of gastroesophageal reflux disease (GERD) is the lower esophageal sphincter rest pressure (LESRP), but these patients can have esophageal motor disorders (EMD). Aim: To assess an association between LESRP and the appearance of EMD in patients with GERD. Subjects and methods: A cross-sectional study was conducted in 229 patients with GERD and 49 healthy controls. Forty five patients with LESRP < 6 mmHg and a mean age of 49 years were assigned to group 1, 128 patients with a LESRP between 6 and 12 mmHg and mean age of 47 years were assigned to group 2, 56 patients with a a LESRP >12 mmHg and a mean age of 47 years were assigned to group 3 and group 4 was conformed by 49 healthy subjects aged 40 years old. Esophageal manometry was performed using previously published techniques. Results: There was a significant association between LESRP, waves amplitude and the frequency of tertiary waves. Conclusions: Resting lower esophageal sphincter pressure is inversely proportional to the presence of esophageal motor disorders in patients with gastroesophageal reflux disease.#Antecedentes: Uno de los factores más importantes involucrados en la fisiopatología de la enfermedad por reflujo gastroesofágico (ERGE) es la presión de reposo del esfínter esofágico inferior (PREI), pero estos pacientes pueden tener trastornos motores esofágicos (TME). Objetivo: Evaluar una asociación entre la PREI y la aparición de TME en pacientes con ERGE. Sujetos y métodos: Se realizó un estudio transversal en 229 pacientes con ERGE y 49 controles sanos. Cuarenta y cinco pacientes con PREI < 6 mmHg y una edad media de 49 años fueron asignados al grupo 1, 128 pacientes con una PREI entre 6 y 12 mmHg y una edad media de 47 años fueron asignados al grupo 2, 56 pacientes con una PREI > 12 mmHg y una edad media de 47 años fueron asignados al grupo 3 y el grupo 4 fue conformado por 49 sujetos sanos de 40 años. La manometría esofágica se realizó utilizando técnicas publicadas previamente. Resultados: Se observó una asociación significativa entre la presión del esfínter esofágico inferior en reposo (LESRP), la amplitud de las ondas y la frecuencia de las ondas terciarias. Conclusiones: La presión del esfínter esofágico inferior en reposo es inversamente proporcional a la presencia de trastornos motores esofágicos en pacientes con enfermedad por reflujo gastroesofágico.
