Association of health knowledge with adoption of heart healthy behaviours: a cross-sectional analysis using data from the PURE study

Aims This study aims to assess aspects of health knowledge: (i) awareness of health effect of tobacco smoking and (ii) awareness of preventive actions for heart disease and stroke, and their relationships with adoption of heart healthy behaviours (smoking cessation and utilization of anti-hypertensive treatment).Methods and results In this study, data from adults aged 35-70 years from 21 countries were included. Data on health effects of tobacco smoking (10 questions) and health actions to prevent heart disease or stroke (11 questions) were collected at baseline. Logistic regression analyses were used to examine the relationship with the outcomes of smoking cessation and use of anti-hypertensive treatment adjusting for adjusting for possible confounders. Of the 12 962 included in the descriptive analysis, 50.0% were female, 42.9% had no or primary education, and 53.3% were residing in low or lower middle-income country. Among current and former smokers, having knowledge of health effect of tobacco smoking on heart disease [adjusted odds ratio (aOR): 1.70, 95% CI: 1.19,2.43], stroke (1.41, 1.08,1.86), and on heart disease in non-smokers exposed to others smoking (1.40, 1.06,1.86) was significantly and positively associated with smoking cessation compared to those who were not aware of health effects. Knowledge of the importance of reducing dietary salt aOR 1.62 (1.23,2.13), dietary fat aOR 1.56 (1.17,2.08), and exercising more aOR 1.48 (1.22,1.80) to prevent heart disease or stroke was positively associated with taking anti-hypertensive medication compared to those who were not aware of preventative actions.Conclusion This study reinforces that better health knowledge shapes adoption of heart healthy behaviours such as smoking cessation and taking anti-hypertensive treatment even after accounting for baseline education and wealth. Health knowledge is an important determinant of heart healthy behaviours across diverse educational or socio-economic backgrounds.Knowledge of health effect of smoking on heart disease, stroke, and on heart disease among second-hand smokers was associated with smoking cessation in PURE study.Knowledge of the importance of reducing dietary salt, dietary fat, and exercising more to prevent heart disease or stroke was associated with taking hypertensive medication in PURE study.Objetivos Este estudio tiene como objetivo evaluar aspectos del conocimiento de salud: (i) conocimiento del efecto en la salud del tabaquismo y (ii) conocimiento de acciones preventivas para enfermedades cardíacas y accidentes cerebrovasculares, y sus relaciones con la adopción de comportamientos saludables para el corazón (cesación del tabaquismo y utilización de tratamiento antihipertensivo). Métodos y resultados En este estudio, se incluyeron datos de adultos de 35 a 70 años de 21 países. Se recopilaron datos sobre los efectos en la salud del tabaquismo (10 preguntas) y acciones de salud para prevenir enfermedades cardíacas o accidentes cerebrovasculares (11 preguntas) al inicio. Se utilizaron análisis de regresión logística para examinar la relación con los resultados de la cesación del tabaquismo y el uso de tratamiento antihipertensivo ajustando por posibles factores de confusión. De los 12 962 incluidos en el análisis descriptivo, el 50,0% eran mujeres, el 42,9% no tenía educación o tenía educación primaria, y el 53,3% residía en un país de ingresos bajos o medios bajos. Entre los fumadores actuales y exfumadores, tener conocimiento del efecto del tabaquismo en la salud sobre las enfermedades cardíacas [odds ratio ajustado (aOR): 1,70, IC del 95%: 1,19, 2,43], los accidentes cerebrovasculares (1,41, 1,08, 1,86) y las enfermedades cardíacas en no fumadores expuestos a otros fumadores (1,40, 1,06, 1,86) se asoció de manera significativa y positiva con el abandono del tabaquismo en comparación con aquellos que no eran conscientes de los efectos en la salud. El conocimiento de la importancia de reducir la sal dietética aOR 1,62 (1,23,2,13), la grasa dietética aOR 1,56 (1,17,2,08) y hacer más ejercicio aOR 1,48 (1,22,1,80) para prevenir enfermedades cardíacas o accidentes cerebrovasculares se asoció positivamente con la toma de medicamentos antihipertensivos en comparación con aquellos que no estaban al tanto de las acciones preventivas. Conclusión Este estudio refuerza que un mejor conocimiento de la salud moldea la adopción de comportamientos saludables para el corazón, como dejar de fumar y tomar tratamiento antihipertensivo, incluso después de tener en cuenta la educación y la riqueza iniciales. El conocimiento de la salud es un determinante importante de los comportamientos saludables para el corazón en diversos antecedentes educativos o socioeconómicos. El conocimiento del efecto en la salud del tabaquismo sobre las enfermedades cardíacas, los accidentes cerebrovasculares y sobre las enfermedades cardíacas entre los fumadores pasivos se asoció con el abandono del tabaquismo en el estudio PURE. El conocimiento de la importancia de reducir la sal dietética, la grasa dietética y hacer más ejercicio para prevenir enfermedades cardíacas o accidentes cerebrovasculares se asoció con la toma de medicamentos antihipertensivos en el estudio PURE.
